A65-year-oldmanpresentedwitha2-weekhistoryofworseningvertigo.Thesensationofvertigowasconstantandexacerbatedbyturninginbed,sitting,andstanding.Exacerbationslasted5–10minutesandwereintenseenoughtocausegaitimpairment.Twodayspriortopresentation,thevertigohadworsenedinseverityandwasassociatedwithoneepisodeofnauseaandvomiting.Reviewofsystemswaspositivefora3-monthhistoryofawhistlingnoiseinthepatient’sleftear,a2-weekhistoryofacute-on-chronicleft-sidedhearingimpairment,andarecentupperrespiratoryinfection.

患者65岁男性，因恶性眩晕2周就诊，患者眩晕呈持续性存在，转身时加重，无论是平卧、坐位和站立位。加重期持续5-10分钟，程度强烈时伴随步态障碍。就诊前两天，眩晕程度加重，并出现恶心、呕吐一次。系统回顾：近3个月患者左侧鸣笛样耳鸣，2周前出现慢性左耳听力下降急性加重，近期有上呼吸道感染史。
